Full Job Description
Are you passionate about supporting people with sight loss to live more independently, confidently and safely? We are looking for motivated and skilled Rehabilitation Workers to join our integrated Visual Impairment Team within Adult Social Care and Health. This is a rewarding opportunity to make a real difference by helping people with sight loss regain choice, control and independence in their everyday lives. In this role, you will assess people's functional abilities and needs, develop tailored rehabilitation programmes, and provide practical skills training such as travel, kitchen, communication and daily living skills. You will work directly with individuals, their families and carers, and a range of professionals to ensure support is personalised, safe and responsive. You will also work alongside colleagues to screen referrals for the Visual Rehabilitation and Hearing Impairment services, making contact with referrers and citizens to gather key information. This will help ensure accurate records are maintained and that referrals are prioritised appropriately, so citizens receive the right support at the right time. You will play an important part in promoting the work of the Visual Impairment Team, maintaining accurate records, contributing to reviews and case conferences, and supporting high-quality, inclusive service delivery.
We are looking for someone who has:
- A DipHE in Rehabilitation Work (Visual Impairment) or DipHE in Rehabilitation Studies.
- Experience of working with people with sight loss in a rehabilitation context.
- Experience of carrying out assessments and reviews.
- Strong communication skills and the ability to adapt your approach to individual needs.
- The ability to plan rehabilitation support, assess risk and manage a varied workload.
- Registration with the RWPN, with acknowledgement on the register that evidence has been provided. You will be forward-thinking, flexible and committed to ongoing learning and development. You will understand the barriers faced by people with sight loss and be committed to anti-oppressive, anti-discriminatory and inclusive practice.
