An exciting opportunity has arisen for a confident proactive individual within Tower Hamlets Community Health Teams. A rehabilitation support worker is required to assist physiotherapists/occupational therapists in providing treatment to patients. The post holder will be responsible for carrying out rehabilitation programs with patients, as devised by the physiotherapist/occupational therapist, to maximise patient's mobility and independence in daily activities in their own home., o Responsible for implementing and supporting rehab plans written by Physiotherapists and Occupational Therapists.

o To involve clients and carers in the planning of care/rehabilitation programmes and encourage self-management where appropriate.

o Contributes to client goal planning sessions and shows initiative in areas where competent and as agreed by senior staff.
