Full Job Description

Do you want to make a real difference in people's lives while developing your clinical skills in a rewarding, community based respiratory team? We are looking for a motivated, adaptable, and proactive individual to join the Brent and Harrow Community Respiratory Service. This is a great opportunity to be part of a dynamic and supportive team that places patients at the heart of everything we do. What the role involves:

  • Supporting registered nurses and healthcare professionals to deliver high quality patient care
  • Taking responsibility for delegated aspects of patient care, including supporting assessments and home visits across a range of community settings
  • Monitoring patient progress and contributing to care plans within your scope of practice, This role combines patient facing care with administrative support within the Brent and Harrow Respiratory Service, helping to improve outcomes for patients with respiratory conditions.
  • You will support the delivery of high quality care, managing a delegated caseload and assisting with assessment and treatment programmes, including pulmonary rehabilitation and oxygen services.,
  • Supporting patient care and promoting independence and self management
  • Assisting with pulmonary rehabilitation and oxygen service delivery
  • Managing referrals, responding to calls, and maintaining accurate patient records
  • Coordinating clinics and supporting service delivery to meet targets
  • Working closely with a multidisciplinary team including physiotherapists, nurses, consultants, GPs and social care teams
  • This is a great opportunity to work within a supportive team while developing your clinical and service experience.

  • A passion for patient centred care and improving health outcomes
  • Experience in a healthcare setting or transferable skills
  • Strong communication and interpersonal skills
  • A full driving licence and access to a vehicle, as the role involves travelling across multiple community locations and this will support you in settling into the role effectively
