Join Our Team as a Rehabilitation Assistant - Tri-Borough Falls Prevention Service Are you passionate about making a real difference in people's lives? We are looking for a dedicatedRehabilitation Assistantto join our acclaimedTri-Borough Falls Prevention ServicewithinCLCH, supporting residents of Westminster, Kensington & Chelsea and Hammersmith & Fulham who are at risk of falls. As a valued member of our team, you will work closely withPhysiotherapists, OccupationalTherapistsand fellow Rehabilitation Assistants, with guidance from yourTeam Lead and Clinical Lead. You will play a pivotal role in deliveringstrength and balance classes, while also working independently in clinic settings toteach exercises,monitormemory, balance, fear of fallingand measure blood pressure. This role also offers the rewarding opportunity towork directly in clients' homes, helping them improve mobility, balance,functionand safe use of equipment under therapist guidance. Comprehensive training on outcome measures and screening tools will be provided, enabling you toconduct assessments confidently and independentlyin both community and clinic settings. - Provide both general and specialist support to patients under the guidance of a registered allied health professional.
- Work across a variety of settings, includingcommunity centres,clinicsand patients' homes.
- Develop competence throughtraining and competency assessments, promotingindividualised, client-centredcare.
- Support patients in achievingoptimumindependence and function, ensuring a positive and inclusive care experience.
- Assist the Falls Prevention Team in achieving key performance indicators whilemaintaininghigh-quality,safeand accessible services.
