Full Job Description
An exciting opportunity has arisen for a confident proactive individual within Tower Hamlets Community Health Teams. An experienced rehabilitation support worker is required to assist Community therapy team in providing initial assessment of patients within set parameters, supporting therapists in organising and running patient exercise groups and implementation of rehabilitation programmes in order to facilitate clients reaching their optimum level of independence and social integration.
The service has a strong and well established training and education programme, along with robust personal development review programme to support staff with their learning and development., 1. To support the department and organisation by carrying out any other duties that reasonably fit within the broad scope of a job of this grade and type of work.
1. To have responsibility for the health, safety and welfare of self and others at work.
1. To represent the profession and the Trust in a positive and professional manner at all times, contributing personally and professionally to the development of the Community Health Services in East London Foundation Trust to the best of your ability.
2. To comply with the requirements of the Data Protection act
1. To comply with the trust's corporate and local policies and procedures on such subjects and clinical management and human resources
2. To ensure that patients, clients and members of the public are consulted and involved in decisions about local health service developments.
3. To undertake such other duties as may be required from time to time and are consistent with the responsibilities of the role and as qualified and able

1. To establish a therapeutic relationship and communicate effectively with clients and carers to implement treatment plan with sufficient explanation to ensure informed consent.
1. To use good communication skills e.g. empathy, persuasion, motivation in difficult circumstances for clients in pain, distress and vulnerable circumstances.
1. To overcome barriers to communication e.g. language, sensory deficits, learning disabilities, anxiety, anger.
1. To work with advocacy and interpreting services for families where English is not the first language in order to ensure effectivecommunication and sensitivity to religious and cultural needs
1. To communicate effectively with all multi-disciplinary staff, when discussing professional and clinical issues. See attached JD for details
