Full Job Description

We're looking for a registered nurse with expertise in palliative and end of life care, to join our community team, to provide specialist palliative care to patients and their families and carers in the community. We're looking for a compassionate, proactive registered nurse to join the Phyllis Tuckwell Hospice at Home Team. Youll deliver the highest standards of palliative and end of life nursing care to patients, their families and carers in the communityproviding holistic assessment, identifying needs, planning care and delivering timely interventions in partnership with Phyllis Tuckwell multidisciplinary team and colleagues across health and social care.   • A successful Registered Nurse will have:
  • RGN Degree Level Qualification
  • Current NMC registration in adult or general nursing
  • Palliative / end of life care or long-term condition experience.
  • At least one year of working as a registered nurse with demonstrable competencies related to end-of-life nursing care
  • Car driver and owner
  • Experience of multidisciplinary team working and with external partners
  • Able to make autonomous decisions demonstrating decision-making, critical thinking and proactive caseload management

    We are based in Farnham and Guildford, and provide bespoke, compassionate palliative and end of life care for people living with an advanced or terminal illness, across West Surrey and North-East Hampshire.
