Registered Nurse Band 6 - Wiltshire

The job is for a Band 6 Registered Nurse/Paramedic to join the Hospital at Home Service in Bath and North East Somerset, Swindon, and Wiltshire. The service is undergoing transformation, creating opportunities to contribute to innovative models of care and service improvement. The role involves delivering high-quality acute care outside traditional hospital environments, working with a multidisciplinary team across various services to provide assessment, treatment, and ongoing clinical management for patients requiring hospital-level care at home. The responsibilities include working independently, coordinating interventions for people who might require hospital care or need support to leave emergency departments safely, and managing a clinical caseload. The position requires a compassionate and proactive clinician who can make clinical decisions and deliver safe, person-centred care.
