Registered Nurse

We are seeking a motivated and compassionate Registered Nurse to join our Out of Hours Nursing Team, delivering high-quality unplanned care to patients across the Hull and East Riding area. In this rewarding role, you will assess, plan, and provide nursing care to patients within their own homes and community settings, helping to meet urgent healthcare needs outside of normal working hours. What you'll do:

• Assess patients' needs and deliver safe, person-centred nursing care in line with care plans.
• Triage, assess and provide unplanned care to support patients in the community and help prevent hospital admissions
• Work with multidisciplinary teams and partner organisations to coordinate effective care and referrals
• Maintain accurate clinical records and contribute to high standards of patient safety and care quality
  
  Registered Nurse with current NMC registration
• Excellent clinical assessment and communication skills
• Commitment to delivering patient-centred care
• Ability to work independently and as part of a multidisciplinary team
• Full UK driving licence and access to a vehicle suitable for the role
