Are you a Registered Nurse who is looking to make a difference in a new team or seeking a new challenge to help support patients in the community?, We are looking to recruit a registered nurse to work within the Amber Valley Community nursing service. . We are friendly, supportive teams who work closely together to meet the needs of the housebound population within Amber Valley. To assess, plan, implement and evaluate individualised patient care promoting self care wherever possible in an efficient and clinically effective way, whilst maintaining privacy and dignity is vital. Providing advice, guidance and support to junior staff in the delivery of high quality care and actively supporting the District Nurse/Senior Community Nurse in the management of the health care environment are integral to this role..   • Experience of working in the community
  • Evidence of research based/reflective practice, Long Term Conditions experience
  • Wound management experience
  • End of Life Care experience
