To assess, plan, implement and evaluate individualised patient care promoting self care wherever possible in an efficient and clinically effective way, whilst maintaining privacy and dignity is vital. Providing advice, guidance and support to junior staff in the delivery of high quality care and actively supporting the District Nurse/Senior Community Nurse in the management of the health care environment are integral to this role.. • Experience of working in the community
• Evidence of research based/reflective practice, Long Term Conditions experience
• Wound management experience
• End of Life Care experience
