To work within the Hounslow Care Home Support Team to provide care and support CQC registered Care Home in Hounslow. The Registered Nurse will work as part of the Care Home Support Team supporting the Care Homes to deliver high quality, person centred, compassionate care. They will be part of the multi-disciplinary/integrated team delivering care and support to patients working within the standards of proficiency outlined by the Nursing and Midwifery Council and in accordance with WLT policies and protocols, with access to advice and supervision as required. To carry out clinical duties and be prepared to undertake clinical skills update as per needs of the service and specific patient after training and competence assessment, with support from an identified mentor for example, blood pressure, pulse and temperature recording, urinalysis, blood glucose monitoring, wound dressings, and non-complex assessments and reviews This will involve working independently but as part of the team covering geographical areas inHounslow., We are looking for an enthusiastic Registered Nurse to provide safe and effective support, education and care as a valuedmember of the Hounslow Care Home Support Team. The Registered Nurse will work autonomously within the Care Homes using their enhanced clinical skills., The Candidate Pack provides an overview of the key tasks and responsibilities of the role, and the person specification outlines the qualifications, skills, experience and knowledge required. - Level 1 Registered Nurse- with valid registration with the NMC - RN1 ,RNA, Experience or recognised qualification in areas such as: ?Health Promotion ?Diabetes ?Care of the older person ?Continence ?Palliative care Medical/Surgical Nursing, Experience of working in the community setting
- One years worked post registration experience in a relevant setting, e.g. palliative care, care of the older person, orthopaedics, community services
- Supervision of junior staff, Ability to undertake a shift system
- Good communication skills effective with clients and colleagues
- Able to work without direct supervision
- Use a problem solving approach
- Demonstrates high standard of Infection prevention and control
- Clinical skills - Administration of medications including injections, catheterisation, wound dressings
- Confidence and ability to establish rapport with team, clients and other agencies
- Computer Literacy and keyboard skills
- Practice with the NMC Code of Professional Practice
- Access to a motor vehicle for use at work and UK driving licence or competent cyclist, Ability to converse articulately on current health issues, demonstrate knowledge in: ?Able to explain the concepts of Essence of Care ?Able to explain the concepts of Team Working ?Able to demonstrate knowledge of professional standards including confidentiality and clinical governance ?Understanding of Adult Safeguarding ?Evidence of Continuing Professional Development ?Demonstrates evidence -based clinical knowledge ?Able to demonstrate knowledge of health and safety and personal safety issues, Knowledge of National Service Framework for Older People & other NSF's
