Full Job Description
We are currently in the position to recruit a Registered Nurse to ICAR (Intermediate Care and Rehabilitation) unit. This is an excellent opportunity for an individual with an interest and desire to contribute to the delivery of high-quality nursing care to patients with complex care and rehabilitation needs. ICAR is a 24 bedded nurse led rehabilitation unit, offering medical, nursing and therapy. assessment and providing patient centred care to optimise independence, recovery and patient wellbeing. You will be part of the multi-disciplinary team, consisting of medical staff, Pharmacists, Nurse Practitioners, Physiotherapists, Occupational Therapists, Nurses and Health Care Assistants providing high quality holistic care to patients and support to their families. The team is welcoming, friendly and supportive and the successful applicant will be fully supported. You will be responsible for the delivery of individualised patient care, through assessment, care planning and evaluation of care and be able to make clinical decisions with a sound knowledge base associated with this patient group. You will be expected to maintain agreed standards of care, directly protecting and promoting patients dignity, choices, self-esteem and well-being at all times with compassion and commitment., You will be working within a service that provides in-patient care in a 24 bedded unit, so will be working to a rota which covers 24 hours seven days per week including bank holidays.
- 1st Level Nurse.
- The desire to make a difference in this clinical environment with an understanding of the complexity of frail, elderly patients and those requiring rehabilitation.
- Experience in a hospital/care environment.
- Demonstrable communication skills.
- Comprehensive patient assessment and care planning.
- Direct hands-on nursing care providing or assisting with patient activities of daily living.
- Follow colleagues mobility plans to promote independence and self-care.
- Monitor patient progress and have a maintain a high standard of documentation.
- Good organisational skills.
- The ability to work as part of a team.
