The successful applicant will take the lead of a caseload of patients and be at the forefront of their care/ or be part of the triage team. You will also be expected to work in collaboration with the Operational Team Manager, Clinical Leads/Senior Practitioners and to have close working relationships with the Inpatient Service and Home Treatment Team.

To offer a range of evidence based psycho-social interventions and pro-active risk management approaches to help the service user and their family to reduce and manage symptoms, high risk behaviour and other disabling effects of psychosis and enable, as far as possible, full and sustained recovery.
To ensure CNWL Child Protection Guidelines and Safeguarding Children and Adult procedures are fully understood by self and others. To ensure the safety needs of children and adults are met at all times and take appropriate action when a child or adult is felt to be at risk or in need.

To engage the service user and their family in relapse prevention and crisis planning, provide, and coordinate interventions that maximise the service users' ability to resolve crises, remain at home and avoid the need for hospitalisation.

The North Kensington & Chelsea Community Mental Health Hub (KCCMHH) , based in Woodfield Road is looking for a passionate Registered Mental Health Nurse who is looking to make a difference whilst working in an environment that promotes staff development and training opportunities.
CNWL is passionate about developing its own staff, driving them to go on to become more established practitioners and this post gives you the room to do so!
KCCMHH is aligned to two local Primary Care Networks (PCN). Working in this way allows the Hub to have a core structure and function to meet the needs of the local population, aligned with our primary care colleagues . T
