Full Job Description
As a Regional Family Finding Practitioner within the Central Regional Permanency Hub, you will work effectively with children and families to secure improved outcomes. Working together with the Together4Children Permanency Coordinators, you will support the coordinators to work in collaboration with Children's Social Workers to achieve timely, well planned adoptive placements for Looked After Children and will undertake a range of functions relating to Permanency Support.Main Responsibility Your main responsibilities will be to,
- Actively support Regional Permanency Coordinators in delivering effective Family Finding across the Together4Children regional footprint, working collaboratively with Children's Social Care Teams across partner authorities.
- Deliver Family Finding activity using a whole-family, permanency-focused approach, contributing to a comprehensive range of Permanency Services and holding a caseload of complex children and families where required.
- Undertake purposeful direct work with Looked After Children and Young People.
- As part of the Central Regional Permanency Hub, act as a key worker for allocated Family Finding cases, working alongside Senior Social Workers and Social Workers to support effective permanency planning.
- Deliver evidence-based group work programmes in relation to Family Finding, as directed by the Principal Manager and Regional Permanency Coordinators.
- Manage cases effectively from referral to outcome, ensuring timely, high-quality casework, accurate recording and compliance with Together4Children Regional Permanency policies, procedures and guidance.
We are looking for you to, - be a confident and child-focused practitioner with experience of working with Looked After and previously Looked After Children
- have a strong commitment to achieving positive permanency outcomes
- ideally, be skilled in Family Finding and direct work with children and families
- be able to manage complex cases and work effectively across agencies.
- bring a trauma-informed, whole-family approach, strong communication skills and a commitment to high-quality practice, recording and timely outcomes.
