Full Job Description
The post holder will work with adults and their families with acute and complex mental health problems. They will provide support, give time in order to promote recovery from ill health and encourage the development of independence. They will be a part of a multidisciplinary team from which they will receive support and supervision, and will work with a group of clients, helping to develop & implement programmes of care within the framework of the City Wide Home Treatment Team, utilising Collaborative Care Planning., Implement agreed programmes of care with service users and carers under the supervision of a named professional.
Help to identify changes to an individual's mental health and have a good understanding and awareness of issues around risk.
To support service users in achieving their goals, by providing practical health and social care agreed by the client and their care co-ordinator
Be able to undertake basic physical health care observations.
Develop a rapport with service users within appropriate boundaries.
To take a role in health promotion both with service users, carers & other staff.
To take a role in evaluation of care provided
To encourage service users to be fully involved and engaged with their agreed care plan and to access appropriate services
Be able to refer individuals to appropriate ongoing community care services, including health, statutory organisations and voluntary services.
Engage in supervision and appraisal process with qualified team members
To maintain existing skills and develop/receive training in new skills as required to carry out the work of the Trust.
Participate in service requirements for audit/data collection
Participate in training, team away days and induction plans
