Full Job Description

An exciting opportunity has arisen in Torbay Drug and Alcohol Service, for the recruitment of a Recovery Coordinator. You will be part of Torbay Recovery Initiatives which is a partnership with Torbay and South Devon NHS trust, Devon Partnership Trust, Waythrough, and Jatis., The principal purpose of the job is to improve the health & wellbeing of people with a range of substance misuse difficulties. This will be achieved by:

oActive involvement in the delivery of high quality, safe and compassionate Drug and Alcohol Services as an effective member of the multi-disciplinary team.

oImplementing specific review of care packages to individuals and groups within standard protocols, integrated care pathways and recovery principles.

oProviding evidence-based advice and consultancy to patients, family members/Carers within standard protocols for care co-ordination/ care management.

oOrganising and managing a defined caseload under guidance and work as lone workers where appropriate. The post holder will be accountable for their own actions within the delivery of this care.

oSeeking advice and expertise of a registered practitioner when required through the use of regular managerial and clinical supervision in accordance with Trust policy.
