Full Job Description
Our Prevention & Early Intervention Service is recruiting Part Time Reablement Support Workers to join dynamic, person-centred teams within Cambridgeshire. These roles support frail, vulnerable adults on their individual reablement journeys following illness or injury, and work independently in the community, helping individuals to achieve their personal goals and regain daily living skills. You will be supporting individuals recently discharged from hospital, delivering care based on tailored goal plans within their own homes. This includes support with personal care, medication, nutrition, mobility, and general well-being.,
- Enabling individuals to regain their independence through short-term support.
- Working in partnership with individuals to achieve goals in their own homes.
- Responding to emergency situations in the community alongside health colleagues.
- Providing personal care and administering medication where required.
- Monitoring and reviewing progress against support plans.

- Good written and spoken English.
- Awareness of confidentiality, equality, and diversity.
- Ability to work independently in the community.
- A full UK driving licence and access to a car (with business insurance).
- You must provide a full employment history with explanations for any gaps.
- Please note that the salary for this role does not qualify for visa sponsorship and the role is subject to a Full and Enhanced DBS check. Part-time hours available, including weekend work on a rota basis. You will be working across Cambridgeshire in a community-based role.
