Full Job Description

An exciting opportunity has arisen to join the Clinical Imaging team at the West Middlesex University Hospital as a MDT/Cancer Pathway Coordinator. The MDT Coordinator is an essential part of the whole team providing high quality and prompt care to cancer patients.

The post holder will have specific responsibility for the management of target cancer pathway Radiology referrals. This involves working closely with modality leads and booking teams and liaising with clinicians, speciality Cancer Pathway navigators and members of the Multidisciplinary Teams (MDT) throughout the Trust.The main duties of the role will be to cover to provide comprehensive MDT coordination and pathway management and analysis to patients. This is supported by effective pathway monitoring and robust patient tracking list (PTL) management to provide the opportunity to mitigate pathways deviating from timed milestones. In collaboration with diagnostic and clinical services, this ensures a consistent and equitable access for patients and enables the Trust to achieve compliance with the national cancer waiting times performance standards.Excellent communication, organisational and interpersonal skills are essential. You will also need to be flexible, have an appreciation of the
confidential nature of the work and be able to manage workloads. Computer literacy would be desirable as you will be expected to input and extract information from the Patient Administrative System - Cerner Millennium (CRS)., Work closely with modality leads and booking teams, liaising with clinicians, speciality Cancer Pathway navigators and members of the Multi-disciplinary Teams (MDT).

Actively track all Target requests referred to the Radiology department ensuring all Target orders are vetted, booked and reported in the required timeframes.

Liaise with imaging leads if there are capacity issues for booking in target appointments.

Work in a team environment to provide appropriate support and advice to other clerical staff as necessary.

Exercise independent judgement and initiative when problems arise, taking the necessary action to resolve the problems or referring to the appropriate person.

Phone patients to make appointments provide instructions, reminders and information where necessary.

Working with colleagues and assisting team members as necessary.

Ensure all patient demographics and contact details are kept up to date on Cerner.

When accessing patient records ensure computerised tracking systems are used appropriately.

Monitor pending referrals vs capacity, liaising closely with the Administrative team to ensure clinics are booked optimally and the service runs smoothly.

Liaise with Consultants and members of the MDT to obtain all relevant information as required.
