Full Job Description
This is an exciting opportunity to join the Community CAMHS North Team as a qualified Children and Young People's Wellbeing Practitioner.
The post holder will be part of the Community CAMHS North team, one of three locality Community CAMHS teams delivering services to children, young people, and their families living in the Bassetlaw, Newark, and Sherwood areas who are registered with a Nottinghamshire GP.
CAMHS provides assessment and treatment for mild to severe emotional and mental health needs for children and young people up to the age of 18.
Children and Young People's Wellbeing Practitioners play a vital role within the team, delivering high-quality, brief, outcome-focused, evidence-based interventions for children and young people experiencing mild to moderate mental health difficulties, including anxiety, low mood and behavioural challenges. The role includes providing one-to-one interventions as well as facilitating small group sessions. As key members of the multi-disciplinary team, they contribute to formulation, assessment, and care planning, ensuring that every young person's needs are understood and responded to effectively.
The post holder will utilise Routine Outcome Measures (ROMs) to support collaborative, goal-focused work with young people and their families, ensuring that interventions remain purposeful and evidence-based., To contribute to the development and delivery of therapeutic interventions within a multi-agency, collaborative framework, supporting children, young people, and families under the supervision of Community CAMHS staff.
Implement agreed interventions in appropriate settings and provide timely feedback and reports to members of the Community CAMHS Team.
Deliver therapeutic interventions and care bundles in innovative and creative ways, offering psychoeducation to children, young people, and their families.
Promote and strengthen the emotional health and wellbeing of children, young people, and families engaged with the Community CAMHS Team.
Attend and contribute to reviews as required, providing clear written and verbal reports on the progress of children and families.
Communicate sensitively, professionally, and supportively with children, families, and partner professionals to ensure needs are fully understood and addressed.
Seek guidance from Clinical and Operational Leads when needed and engage in regular clinical and managerial supervision in line with Trust policy and professional standards.
Support colleagues and contribute to a positive, collaborative team culture.
Maintain accurate, contemporaneous records in accordance with Trust policies and procedures., + Assess and deliver outcome-focused, evidence-based interventions to children and young people experiencing mild to moderate mental health difficulties across community settings and CAMHS services.
+ Work in partnership with children, young people and their families to support the self-management of presenting difficulties.
+ Collaboratively develop intervention plans and agreed outcomes with children, young people and families.
+ Support and empower children, young people and families to make informed choices about their intervention.
+ Always operate from an inclusive values base that recognises and respects diversity.
+ Undertake accurate assessment of risk to self and others.
+ Adhere to service referral protocols and signpost unsuitable referrals to appropriate services as required.
+ Through case management and supervision, escalate cases that exceed the scope of the role or indicate increased severity, ensuring alignment with service processes.
+ Provide information and support for evidence-based psychological treatments, which may include guided self-help delivered face-to-face, by telephone or via other media.
+ Adhere to the agreed activity contract regarding the number of contacts and sessions offered per week to support timely access and reduce waiting times
+ Attend multidisciplinary meetings related to referrals or children and young people in treatment, where appropriate.
+ Maintain coherent and compliant records of all activity in line with service protocols, using documentation and outcome data to inform clinical decision-making.
+ Work collaboratively with a range of relevant professionals when indicated.
+ Work alongside children, young people and communities to enhance engagement and widen access to services.
+ Identify and manage risk, liaising with other professionals to ensure timely and appropriate responses.
+ Observe and adhere to the Community CAMHS Service Specification, vision, aims and objectives.
+ Provide psychoeducation and information on appropriate self-help materials to professionals, children, young people and families.
+ Adhere to safeguarding policy, procedures and all relevant national and local legislation.
+ Seek support from colleagues, clinical supervisors and CAMHS team leads as required, and engage in regular clinical and managerial supervision in line with Trust policy.
+ Support peers and colleagues and contribute to a positive, team-working environment.
+ Ensure suitable risk management and care plans are formulated for all clinical cases.
+ Work in partnership with other teams across Nottinghamshire and bordering CAMHS services
Training and Consultation:
+ Participate in the delivery of a range of training programmes for staff, parents, children and young people.
+ Support colleagues and stakeholders in addressing and reducing stigma related to mental health.
+ Be willing to undertake training aligned with the CYP IAPT agenda and other development needs identified through appraisal.
+ Attend multi-agency forums to provide consultation regarding individual cases
Liaison:
+ Ensure effective liaison between relevant professionals involved in supporting children and families, to promote the emotional health and well-being needs of children and young people.
+ Support effective multi-agency partnership working and enhance accessibility to services that address the emotional health and well-being needs of children and young people.
Professional:
+ Maintain professional registration and adhere to all relevant professional codes of conduct, practice standards and guidelines.
+ Participate in the appraisal process, identifying personal development needs and actively seeking opportunities for ongoing professional growth.
+ Maintain accurate, up-to-date records in accordance with Trust policies and procedures.Actively promote the service and its reputation, supporting wider understanding of the team's work and responsibilities.
+ Keep comprehensive clinical notes and prepare formal documents, including letters, reports and summaries in line with Trust policies and procedures.
+ Record and submit statistical information as required by the service for reporting to both local team systems and the Trust's data collection processes (MHSDS).
+ Maintain expected levels of clinical activity with children and young people.
+ Ensure all administrative tasks are completed within the timeframes outlined in service protocols.
+ Attend and participate in team meetings, business meetings, supervision and support meetings as required.
+ Follow Trust policies and procedures, maintaining up-to-date skills and knowledge through ongoing participation in continuing professional development
Clinical Governance:
+ Participate in service audits, reviews and risk assessments.
+ Contribute to ensuring that all practice is clinically safe and demonstrably evidence-based.
+ Comply with Trust guidelines regarding clinical supervision, professional development programmes and staff evaluation processes.
+ Attend and participate in fortnightly case management supervision.
+ Undertake the range of qualitative and quantitative outcome measures outlined within the service specification to support quality assurance.
+ Contribute to the ongoing development of the low-intensity pathway within CAMHS.
+ Promote a culture of continuous quality improvement, consistently striving to enhance the standard of care provided to children and families.
+ Take appropriate steps to improve service-user feedback and engagement in service design, delivery and evaluation.

+ Participate in safeguarding children supervision.
+ Undertake the minimum safeguarding children training requirement of one full day per year, alongside any additional courses or seminars as needed.
