We are delighted to offer an exciting opportunity to join our nationally accredited, friendly and dedicated Pulmonary Rehabilitation team at County Durham and Darlington NHS Foundation Trust (CDDFT) as a Band 5 physiotherapist.
The Pulmonary Rehabilitation Service is part of the Community Care Group and is committed to delivering high-quality, evidence-based rehabilitation programmes to patients across County Durham and Darlington. Our aim is to support patients in managing their respiratory conditions such as COPD, bronchiectasis and pulmonary fibrosis, enhancing their quality of life and promoting self-management through tailored exercise and education.
This is a temporary fulltime position fixed term until 31/3/2027. The PR service deliver rehab countywide, and the successful candidate will be expected to travel throughout the county as the service demands.
Applications are welcome from Physiotherapists who are newly qualified or due to qualify, with HCPC registration required prior to appointment.
Candidates are exepcted to have a thorough understanding of pulmonary rehab., This role supports the delivery of a comprehensive, community-based Pulmonary Rehabilitation (PR) exercise programme for individuals living with Chronic Obstructive Pulmonary Disease (COPD) and other long-term respiratory conditions.
Under the supervision of the Band 7 Clinical Lead (PR), you will:
Assess, design, implement, and evaluate personalised pulmonary rehabilitation exercise and education programmes, using evidence based practice and patient centred goals.
Monit or and review patient progress, adapting exercise plans as needed in collaboration with the multidisciplinary PR team.
Assist in managing waiting lists and facilitate appropriate referrals to other services when required.
Provide specialist guidance and education about the PR service to healthcare professionals across sectors, supporting effective collaboration and seamless patient care.
Produce clear, comprehensive reports for general practitioners and other referrers, summarising assessment findings and discharge outcomes following completion of the PR programme.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
