Full Job Description

The role will be to support the delivery of the Healthy Child Programme (HCP) 0-19 Thereby improving outcome for children and young people. To contribute to the Public Health agenda, meet Public Health targets and reduce health inequalities in partnership with key agencies meeting National agendas. To promote the protection of children, adhering to the Local Safeguarding Children's Partnership polices and procedures.
To work under the supervision of a band 6 Health Visitor/School Nurse within a designated area, along side partner agencies such as Action for Children, The Children's Society, Family Hubs, Children's Services, and education systems. To work as part of a team to ensure that Key Performance Indicator (KPI's) targets for the service are met. To support service development, standard setting, clinical audit and bench marking, deliver a high quality needs led, safe and effective service to all client groups., · Participate in the assessment of health needs and formulation of care plans for children, young people, their families and their communities. The includes the implementation of programmes of care, offering support and advice to parents and families in the home, clinic school or community setting.
+ In collaboration with the public health nursing team, identify individuals and groups within the community where collaborative working and advice would forward the interests of public health and promote healthy living, in particular seeking to identify disadvantaged groups and address health inequality Offers advice and support to children, young people and adults in accordance with evidence-based practice.
