Full Job Description
As a Community Nurse within our 019 Healthy Child Programme, you will play a key role in shaping how we support children and families to thrive. Working as part of an innovative Family Hub model, you will focus on strengthening the home learning environment, improving engagement with families and increasing attendance and outcomes from developmental reviews. This is a highly collaborative role, where you will build strong relationships with Specialist community public health nurses, early years settings, education and wider partners to ensure children are identified early and supported through the right pathways at the right time. You will bring curiosity, initiative and a passion for improving outcomes, helping to reduce inequalities, address missed opportunities and support more children to achieve a Good Level of Development., In this role, you will take a proactive, outreach-focused approach to engaging children and families who are not accessing universal services or early years provision. You will work across community, home and partner settings to understand barriers to engagement, using relationship-based practice to build trust and improve attendance at developmental reviews. You will play a central role in strengthening early identification, using professional curiosity, data and partnership intelligence to identify children who may be at risk of poorer outcomes. Working closely with early years, education and multidisciplinary partners, you will support timely assessment, escalation and intervention, ensuring children are on the most appropriate pathway as early as possible. A key part of the role will involve closing gaps in developmental outcomes by targeting those children who are not routinely seen or whose needs are not yet understood. You will contribute to improving the quality and coverage of developmental reviews, reducing missed opportunities and supporting more children to achieve expected levels of development. You will also help shape and embed new ways of working across the Healthy Child Programme and Family Hub network, contributing to service improvement, pathway development and collaborative approaches to care.
