Full Job Description
An exciting opportunity has arisen to recruit a Band 4 Care Coordinator to join the busy Prostate Clinical Nurse Specialist team. We are looking for a highly motivated, organised and compassionate person to join our team at the Royal Surrey County Hospital. The post holder will support the delivery of the Prostate Cancer Supported Self-Management (SSM) pathway, working closely with the Prostate CNS team to ensure safe, effective, and patient-centered care.
They will act as a key point of contact for patients on remote monitoring pathways, coordinate PSA tracking and safety-netting processes, and provide high-quality information, support, and signposting. The role involves the completion of holistic assessments, Implementing personalised care plans and signposting patients, carers and their families to support agencies highlighted through the assessment process, supported by the CNS team., The post holder will support the delivery of the Prostate Cancer Supported Self-Management (SSM) pathway, working closely with the Prostate CNS team to ensure safe, effective, and patient-centered care.
They will act as a key point of contact for patients on remote monitoring pathways, coordinate PSA tracking and safety-netting processes, and provide high-quality information, support, and signposting.
The role enhances patient experience, improves pathway flow, and supports the CNS team by undertaking delegated clinical and administrative tasks.
The postholder will be an integral part of the Oncology prostate team, assisting the Clinical Nurse Specialist (CNS). The main purpose of the role is to support people to navigate the complex health and social care system during and/or following completion of cancer treatment. The post holder will work autonomously within appropriate occupational guidelines. They will refer to clinical nurse specialist team to support patients with self-management and follow up following their treatment when necessary. You will coordinate care by delivering emotional and practical support to patients with a diagnosis of cancer. You will be involved in monitoring patients on an active surveillance pathway who require regular blood tests, MRI scans following a treatment protocol. You will also be involved in supporting the prostate support worker in the delivery of Holistic Needs Assessment's which aim to identify and address patient's concerns.
The post requires excellent communication skills and the ability to manage a large patient group often dealing with sensitive information. Previous oncology experience is an advantage. The ability to work on your own initiative and as part of a multidisciplinary team are essential.
