Project Coordinator for Volunteer Discharge Support

Join our friendly, supportive team and play a vital role in helping patients make a safe, timely and positive transition from hospital to home. As Volunteer Coordinator, you will lead and support a diverse team of volunteers delivering services that make a real difference to patients' lives. You will coordinate volunteer drivers providing rapid discharge transport and deliveries of mobility equipment, medications and virtual ward kits, oversee our Settle In volunteers who help patients on the day they return home, and manage volunteers making welfare and safety-netting calls following discharge. You will also help connect patients with appropriate community support services where needed., You will coordinate the day-to-day operation of our hospital volunteer services, ensuring patients receive safe, timely and compassionate support throughout their discharge journey. Working closely with ward teams and community partners, you will coordinate referrals, allocate volunteers and monitor service delivery across three volunteer-led services: rapid discharge drivers, delivery of mobility equipment, medications and virtual ward kits; our Settle In service, supporting patients on the day they return home; and post-discharge welfare and safety-netting calls. You will recruit, induct, train and support volunteers, maintaining high standards of practice and ensuring volunteers feel valued, confident and well supported. You will work collaboratively with clinical and operational colleagues to continually develop the service, resolve issues and identify opportunities to improve patient experience.

This is a varied, fast-paced and rewarding role where no two days are the same. You'll need excellent organisational skills, the ability to prioritise and think on your feet, and a compassionate, person-centred approach. As this is a job-share position, so strong communication, teamwork and handover skills are essential to ensure a seamless service. We're looking for someone who is caring, adaptable and motivated by improving patient experience. You'll enjoy building relationships with volunteers, colleagues and partner organisations. Must hold a full UK driving license without restrictions.
