Primary Care Quality Improvement Facilitator for Cancer

This post willwill be working in conjunction with the teams at these Places and the Awareness and Early Diagnosis Team of Humber and North Yorkshire Cancer Alliance which works across the cancer pathway and spans the whole ICS.

As one of our team of Primary Care Quality Improvement Facilitators for Cancer you will provide invaluable practical implementation support to PCNs to improve the quality of their services and work towards achieving cancer-related objectives. You will also provide a vital communications channel between primary care and the Alliance work programmes., Humber and North Yorkshire Cancer Alliance have launched a new incentive scheme to support PCNs undertaking actions and improvements across their cancer activities. As one of our team of Primary Care Quality Improvement Facilitators for Cancer you will provide invaluable practical implementation support to PCNs to improve the quality of their services and work towards achieving cancer-related objectives. You will also provide a vital communications channel between primary care and the Alliance work programmes.
