Primary Care Network Pharmacist, Band 8a WISE 2 - (1 Year Secondment)

The post holder will be responsible for:

• Support delivery of medicines related QoF and CQC indicators, local prescribing incentive scheme and medicines safety agenda
• Support the practice with chronic disease management and day-to-day medicines related queries
• Promoting cost effective and evidence based prescribing, following national and local guidelines.
• Supporting the delivery of the pharmacy and medicines optimisation elements of the network service specifications within the Network Contract DES with a particular focus on the structured medications review and optimisation specification and the enhanced health in care homes specification.
• Support medicines optimisation across the interface and within the PCN
• Improve medicines safety and patient outcomes relating to medicines.
• Support in the management of chronic diseases, antimicrobial stewardship and polypharmacy
• To support the networking, education and development of trainee and foundation pharmacist

We are looking to recruit a clinical pharmacist interested in developing their career in primary care, improving patient outcomes, reducing prescribing variation and ensuring patient safety. With the increasing burden of polypharmacy and complex co-morbidities, we are looking for a pharmacist with a keen understanding of balancing risks and benefits in relation to medicines and patient circumstances. The position involves working part time within WISE 2 PCN; at Sunny Meed Surgery as a 1 year fixed term secondment. You will be motivated, and passionate about patient care with excellent communication skills to work with a multi-disciplinary team across the locality to optimise medications with a patient-centred approach. This pharmacist post is a great opportunity for pharmacists wishing to develop their career. You will ideally be a prescriber with experience of working within hospital or primary care.
