Primary Care Network Dietitian

We are looking for an experienced dietitian to provide cover within a Primary Care Network in Shoreditch Park and City PCN. This is an exciting opportunity to work at the forefront of primary care, delivering specialist dietetic services directly within GP practices. As a PCN Dietitian, you will play a vital role in supporting patients with nutrition-related conditions, helping to reduce pressure on GPs while improving access to high-quality dietetic care in the community., You will manage your own clinical caseload, working autonomously within GP practices as part of a wider multidisciplinary team. Your focus will be supporting patients with conditions such as pre-diabetes, type 2 diabetes, weight management, frailty and paediatrics - helping them make lasting changes through person-centred care and behaviour change approaches. You will also have opportunities to contribute to service improvement, clinical audit, and pathway development. Group education sessions may form part of the role depending on service needs.
