Full Job Description

NBT pharmacy are looking to recruit a motivated and enthusiatic team member to join their nationally recognised Pre-Op Surgery team. This part time post will work alongside other MDT members to support patients manage their medications safely in the Pre and Peri-Operative period for our elective surgical specialities in Orthopaedics, Plastics, Vascular, GI surgery and Renal transplantation.
The role also involves the support and management of pharmacists providing cover to our surgical speciality wards and supporting the specialities with governance and clinical expertise. With the imminent introduction of EPMA to the hospital, we are also looking for staff interested in taking this opportunity to review and develop our services in new directions to provide excellent patient centred care., Responsible for the efficient and effective provision, management, and development of the appropriate clinical pharmacy service in accordance with Medicines legislation, the Code of Ethics of the GPhC, RPS hospital pharmacy standards, UKMI standards and Trust policies and procedures.
Provides specialist clinical pharmacy services at a high level to the Pre-Op pharmacy Surgical Team. Teaches and supervises less experienced colleagues, pharmacists, technicians, and students., 1. Responsible for the provision, management and development of a highly specialist pharmaceutical service to the Anaesthetics, Surgery, Critical Care and Renal (ASCR) Division to achieve a high standard of efficiency and effectiveness.
2. Responsible for the provision, management and development of a highly specialist pharmaceutical service to the Pre-Operative Assessment Clinics and managing the pharmaceutical care in the pre and peri-operative period.
3. Clinically screen prescriptions to ensure the safe, efficient, evidence-based and rational use of medicines in line with the BNSSG Formulary and departmental policies so that risks to patients are minimised
4. Contribute to the overall development of the clinical pharmacy service through active participation in relevant meetings. Work closely with all members of the pharmacy department and other healthcare professionals.
5. Attend consultant ward rounds and ward board rounds whenever possible to provide pharmaceutical expertise
6. Demonstrate strong leadership skills across all elements of the role
7. Provide reports on the use and cost of medicines to the surgical directorate as appropriate and analyse these to advise on strategies to ensure cost-effective use of resources.
8. Write relevant protocols, procedures and guidelines for use within Pre-op Surgery clinics and in surgical specialist environments
9. Provide pharmaceutical expertise to any multidisciplinary working groups considering protocols, policies, guidelines and integrated care pathways in the specialist area
10. Assimilate and analyse clinical trial information on new pharmaceutical products intended for use within the orthopaedic speciality for submission to the Area Prescribing and Medicines Optimisation Committee (APMOC)
11. Ensure that an accurate medication history is obtained for each patient using the appropriate sources (GP, community pharmacist, patient/carer, clinic notes, nursing home, electronic patient record systems)
12. Assist with prescribing to support pre-op patients and inpatients to enable appropriate patient flow
13. Deals with high level pharmaceutical queries and provide advice and information to patients/carers/nursing and medical staff in connection with drug treatment
14. Provide a comprehensive clinical ward pharmacy service to the speciality and work with stakeholders to optimise resources and improve patient flow
15. Work closely with internal and external stakeholders, where necessary to ensure timely and seamless discharge of patients
16. Document all major interventions made
17. Investigate and support the recording of adverse drug reactions and support the documentation of patient allergy in appropriate clinical records
18. Audit and risk manage selected areas of practice to promote continuous quality improvement by affecting change in practice. Participate in surgical and pre-op clinical governance and audit meetings
19. Maintain and develop medicines management systems in conjunction with patients/carers, ward staff and pharmacy staff. Supervise and contribute to the development of medicines management technicians and/or other dedicated pharmacy staff
20. Contribute to the training of pharmacy undergraduates, medical students, junior doctors, pre- registration pharmacists, pharmacy technicians, allied health professionals and medical and nursing staff as appropriate. Train and mentor clinical pharmacy diploma students on aspects of the specialist area
21. Act as an appraiser in the Pharmacy appraisals programme
22. Ensure that all relevant procedures are adhered to, reviewed regularly and that all necessary records are completed
23. Undertake to follow all departmental procedures in accordance with current local practices and legislative requirements
24. Provide professional cover in the dispensary on a sessional basis
25. Keep up to date with developments in Pharmacy to maintain a high level of professional knowledge and competence. Attend study days and courses as necessary for the development of the service and the individual
26. Maintain a record of CPD
27. Work with the specialty to comply with national and local patient safety drug alerts and initiatives and help with the implementation and recommendation of associated action plans
28. Provide comprehensive pharmaceutical care to surgical patients in all clinical areas
29. Undertake any other duties as required by the Director of Pharmacy or their deputy
