Full Job Description
We are piloting and developing a new innovative pre-diagnostic hub clinical nurse specialist service and this is your chance as a nurse to be part of its development and make a difference to our patients on cancer pathways., We are looking for a Clinical Nurse Specialist Nurse to provide leadership to our new service and play a key role in it's development. You will support clinical teams by facilitating patient access to various diagnostic tests and support co-ordination of the pathway in accordance with agreed guidance. Tracking and monitor diagnostic tests and results for identified patients, escalating abnormalities to the responsible clinical team. Complete accurate and high standard of documentation of patient interactions and clinical narrative on hospital systems, including Somerset, to effectively communicate, promote safety and collect service data This post is fixed term/secondment until 31st March 2027. Internal applicants currently employed by ESNEFT will be offered on a secondment basis only. Please ensure you have discussed this with your line manager.
- To monitor red flag symptoms of patients on complex or long pathways, flagging any concerns to the responsible clinician, maximising patient safety and outcomes
- To provide information and psychological support, advising patients and their loved ones of expected next steps
- To facilitate transfer of nursing care to the relevant key worker if a cancer is diagnosed, or support discharge back to their GP if benign or otherwise indicate
- To undertake holistic needs assessments of identified patients, enabling timely support and advocacy of patients' personalised care needs
- To be proactive in the setting up and establishment of this new hub model, actively engaging with other stakeholders to ensure that processes are robust and the service operates within the agreed guidance
- Prioritise own workload and manage time effectively
- Ensure that all interactions with patients, relatives, visitors and colleagues is conducted in a polite and respectful manner
- Maintain accurate records of pre-diagnostic interventions and outcomes, ensuring that patient confidentiality is adhered to within the Trust guidelines
- Actively monitor and escalate performance issues regarding Patient Tracking Lists, backlogs and delays
- Establish effective working relationships with other healthcare professionals whose role impinges on the pre-diagnosis nursing service e.g. AOS, GP's and nurse; radiographers, clinicians, cancer nurse specialists, GPs, allied health professionals and community care teams.
