Are you passionate about delivering high-quality, patient-centred care & preventing unnecessary hospital admissions?
We are looking for an enthusiastic & motivated B6 Nurse, Occupational Therapist, Physiotherapist or Paramedic to join our innovative Urgent Response and Frailty Team, working across both the Emergency Dept at University Hospital Southampton & within the community setting.
This is an exciting opportunity to be part of a forward-thinking service supporting the NHS ambition to shift care closer to home. You will play a key role in delivering care that enables patients to receive hospital-level treatment in their own homes, helping to avoid admission where appropriate.
Main duties of the job:
Undertake holistic, complex assessments for patients presenting in ED and community
Deliver clinical monitoring & therapeutic interventions to support recovery & independence
Work flexibly across:
Urgent Response in ED (formerly CEDT)
Southampton Urgent Response Service
Southwest Hampshire Frailty Service
Contribute to hospital admission avoidance and safe discharge planning
Support the development & supervision of junior staff
Work collaboratively within a multidisciplinary team to deliver high-quality care
Contribute to ongoing service development & improvement.
You will be joining the service at an exciting time of development, as we integrate acute & community services across University Hospital Southampton & Community services across Southampton & Southwest Hampshire., To work flexible across the front-door of UHS (Urgent Response Service in ED) and the Community (Southampton and Southwest Urgent Community Response and Hospital at Home Service) as part of an integrated health care team providing assessments and crisis interventions to prevent hospital admissions, and to support hospital discharge service for patients across the City of Southampton and South West Hampshire.
