Full Job Description

The Somerset NHS Foundation Trust Psychiatric Liaison Service provides specialist mental health assessment, intervention and advice for patients presenting to the acute hospital with suspected or actual mental health difficulties. This includes individuals experiencing acute psychiatric states such as psychosis, severe depression, dementia and delirium, ensuring their mental health needs are met alongside their physical care.,

  • Undertake comprehensive biopsychosocial mental health assessments for patients presenting in acute settings
  • Deliver evidencebased interventions for adults experiencing suspected or confirmed mental health difficulties
  • Assess and manage clinical risk, including patients presenting in mental health crisis
  • Support admission avoidance and facilitate timely, safe discharge planning
  • Provide specialist mental health advice and consultation to acute hospital staff
  • Work collaboratively with patients, carers and partner agencies to establish onward community care pathways
  • Maintain accurate and timely clinical records in line with Trust policy
  • Participate in team meetings, clinical governance, audit and service development activities

    We are seeking a motivated and compassionate Band 6 clinician to join our Psychiatric Liaison Team based at Musgrove Park Hospital. This is a rewarding opportunity to work within a fastpaced acute hospital setting as part of a supportive, experienced multidisciplinary team, delivering highquality mental health care 24 hours a day, 7 days a week.
