Practice Nurse - with experience in General Practice F / T or P / T

The post holder will be responsible for delivering safe, effective, and evidence-based nursing care to the practice population. This includes the assessment and management of patients with acute and chronic conditions, as well as providing health promotion and preventative care interventions.

Working collaboratively with the wider multidisciplinary team, the Practice Nurse will support the delivery of high-quality patient care and clinical procedures. The role will also contribute to achieving key performance targets, including the Quality and Outcomes Framework (QOF), Local Incentive Schemes (LIS), Directed Enhanced Services (DES), Childhood Immunisation targets, and any other contractual or organisational objectives.

The successful candidate will have previous experience in General Practice and be qualified to undertake Cervical Screening and Childhood Immunisations. Qualifications in Asthma and Diabetes management would be advantageous.
