Practice Nurse with Chronic Disease Management Experience

Whitstable Medical Practice is looking for a motivated, compassionate and clinically confident Practice Nurse with experience in Chronic Disease Management to join our friendly and supportive multidisciplinary team. This is an exciting opportunity for a nurse who enjoys developing long-term relationships with patients and has a particular interest in the management of Asthma, COPD and/or Diabetes, alongside the wider range of services delivered within primary care. We are dedicated to delivering high quality care to our 45,500 patients across three sites: Estuary View Medical Practice, Chestfield Medical Centre and Whitstable Medical Centre. The successful candidate will be required to work flexibly across all three locations, as required.
