Practice Nurse Weald Primary Care Network

We are seeking an enthusiastic and motivated Band 5 or Band 6 Registered Nurse to join our expanding Single Neighbourhood Team within the Weald Primary Care Network. This is an exciting opportunity to work alongside a multidisciplinary team that includes Nurses, Clinical Pharmacists, GPs, and Care Coordinators, supporting the delivery of high-quality, patient-centred care across our member practices., The successful candidate will play a key role in managing and supporting our frailty cohort, including palliative care patients, housebound individuals, and those living with frailty. The postholder will contribute to the delivery of Comprehensive Geriatric Assessments (CGAs) and work collaboratively with GPs and the wider multidisciplinary team to coordinate ongoing care and treatment.

The role will also involve delivering a range of nurse-led clinics and Housebound and Care Home visits, including vaccinations.

We welcome applications from experienced Band 5 nurses looking to develop within primary care, newly qualified nurses with a strong interest in community and frailty care, and Band 6 nurses seeking a varied and autonomous role. The ideal candidate will be confident working independently across the PCN, demonstrate excellent communication skills, and be committed to collaborative multidisciplinary working to improve outcomes for patients.
