Practice Nurse

As a Practice Nurse, you will be responsible for the delivery of practice nursing services care to the practice population. Supported by the Lead Nurse and General Practitioners within the practice, you will deliver care within the boundaries of your scope of practice, focusing upon supporting patients to be healthy, monitoring and management of long-term conditions, health prevention, wound care and screening activities that may include cervical screening, baby immunisations and travel health. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures. The working pattern for this role is Thursdays and Fridays, with a 7:30am start on one of those days. The rate of pay is dependent on the following competencies

• Including thorough understanding of QOF requirements / enhanced services / contribute to audits / governance structures / leaning from incidents / active participant in nursing and clinical meetings
• Undertake assessment and interventions (ear irrigation)
• Understanding of screening programmes, practice uptake and contribute to maximising uptake in practice population
• Cervical screening: Undertake procedure, follow practice protocol, interpret results, audit results
• Understand national schedules, administer from PGD or PSD as appropriate, ability to fully meet patients needs for information and guidance, Work autonomously and efficiently
• Support GPs and ANPs to help improve efficiency in GP and ANP workload, allowing GPs to focus on more complex care.
• Protection and potential enhancement of QOF and enhanced service income
• Assess, plan, develop, implement, and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being.
• Implement and evaluate individual treatment plans for patients with a known long-term condition.
• Support and manage health needs of women presenting for cervical cytology consultations.
• Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition. Including Diabetes, COPD, Asthma, Hypertension, CVD and other conditions.
  
  Registered first level nurse (NMC)
• Evidence of recent CPD and training
• Experience of working in General Practice
