Practice Nurse

An exciting opportunity has arisen for an enthusiastic and motivated Practice Nurse to join our vibrant, forward-thinking practice. The successful candidate will be an integral part of the nursing team and will be responsible for participating in the delivery of high-quality patient care to our registered practice population., The focus of the role is both the delivery of evidence-based practice for patients presenting with a long term condition, and the provision of preventative health care to the practice population. You will deliver care, focusing on supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively within the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

Working alongside our Nurse Manager, three Practice Nurses, and two GPAs; we are ideally looking for chronic disease experience in Diabetes Management.

The practice has over 16,000 patients, and is at the forefront of new care models and network working. We have a strong ethos on continuous learning and offer training opportunities and clinical support together with an enthusiastic multi team approach to care. (Specialist nurse led clinics, urgent care team, health coaches, GPAs, nurse training mentorship).
