Full Job Description

- conduct comprehensive assessments of patients health needs
- develop implement and review individualised care plans
- provide appropriate treatments and interventions as required
- undertake treatment room responsibilities and deliver chronic disease management clinics
- carry out specific clinical tasks including but not limited to + travel health consultations and vaccinations + cervical screening + childhood immunisations + wound management + blood pressure monitoring and ecgs + venepuncture and health checks
- work collaboratively with gps other nurses and the wider healthcare team to ensure high quality patient care
- maintain accurate timely and confidential patient records in line with nhs standards
- deliver health education advice and counselling to patients and their families
- keep up to date with current clinical guidelines medical advancements and best practice in primary care
We are looking for a qualified General Practice Nurse, and nurses with relevant experience who are keen to develop their skills in primary care are encouraged to apply.
