This is an exciting opportunity to play a key role in delivering our Health and Employment Integration programme, including the national WorkWell programme, helping people with health conditions stay in, return to, or access work. Working across the NHS, local authorities, employers, the voluntary sector and wider partners, you'll help turn ambitious population health strategy into practical action that improves lives and reduces health inequalities.

No two days are the same. You'll coordinate complex programmes, analyse and interpret data, work with a wide range of partners, support programme governance and help shape innovative approaches that address the wider determinants of health.

As a Population Health Officer, you will play a central role in supporting the delivery of our Health and Employment Integration programme, including the national WorkWell programme, helping to improve health outcomes, reduce health inequalities and support more people to stay well and in work.

Our Population Health Team sits within the Chief Medical and Neighbourhood Directorate and provides system leadership across prevention, health inequalities, population health management and learning and change. We work with colleagues across the NHS, local authorities including Public Health, primary care, voluntary organisations, employers and communities to tackle the causes of poor health, improve outcomes and reduce inequalities for more than two million people.

We're looking for an enthusiastic and motivated Population Health Officer to join our innovative Population Health Team within the Chief Medical and Neighbourhood Directorate at NHS Hampshire and Isle of Wight.
