PN Transformation Lead Pharmacist

The Parenteral Nutrition (PN) Transformation Lead Pharmacist acts alongside the Nutrition Consultant Pharmacist as an expert within the Adult Nutrition Team to provide a dynamic link between clinical practice and service development by creating new models for delivering patient care ensuring the best experience and outcomes for patients from their PN. The post will work with existing teams to lead and deliver implementation of the PN improvement Programme across adult, children and neonate services. The focus will be to improve ordering efficiencies resulting in reduced spend and to providing evidence and assurance of progress towards implemented change., The post holder will drive the transformational change programme to deliver efficiencies, linking closely with clinical, admin and digital colleagues to determine and implement the proposed changes. You will be a credible expert in PN, have excellent proven project management skills and be a great people person. Reporting back on progress will also be a key deliverable.., 2. To lead implementation of the PN improvement programme across a range of strategies including increased pre-ordering, appropriate use of standard off-the-shelf PN bags, restricted formulary development for less complicated Type 1 PN patients, and reduction of avoidable waste, duplicate orders and late ordering charges. 3. To work across Divisions, CSS Pharmacy, Digital, Finance, Procurement, Commissioning and Clinical nutrition teams to deliver a coordinated programme of change with clear governance, milestones, benefits tracking and risk management. 4. To build effective operational processes that improve accountability for PN expenditure, improve invoice reconciliation and coding accuracy, and support delivery of forecast Cost Improvement Programme and invest-to-save benefits. 5. To support safer and more timely PN provision by reducing delays in treatment, improving ordering discipline and reducing delayed discharges associated with PN ordering or supply processes. 6. To lead engagement with adult, paediatric and neonatal PN stakeholders, including intestinal failure teams, surgery, critical care, urology, transplant, paediatric and neonatal services, ensuring service changes are clinically appropriate and operationally deliverable. 7. To develop and implement standardised PN ordering, approval, formulary and monitoring processes that support appropriate patient selection, safe prescribing and consistent supply arrangements. 8. To oversee the delivery of digital improvements required to create an end-to-end PN ordering, tracking and reporting process, working with pharmacy digital colleagues and operational users to ensure safe adoption. 9. To support and deputise for the Consultant Nutrition Pharmacist to manage and strategically lead and co-ordinate the adult nutrition pharmacy team in the provision of a safe, effective and efficient clinical pharmacy service to the Gastroenterology, Endoscopy & Theatres (GET) Directorate, in accordance with local and national standards and strategy. 10. To ensure the safe and effective use of PN across the Trust through provision of medicine related aspects of patient care, promoting economy and improvements in quality, safety and cost effectiveness, and medicines optimisation. 11. To provide leadership, supervision and workstream coordination for project resources, including pharmacy, digital and analyst support, ensuring roles, outputs and dependencies are clear. 12. To produce regular reports for the SRO, divisional leadership, finance, pharmacy governance groups and relevant committees, demonstrating progress against financial, workforce, activity, quality and safety metrics., The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.

• Degree in Pharmacy (Masters) or equivalent
• Current registration with the General Pharmaceutical Council
• Post-Graduate Clinical Pharmacy Diploma or equivalent experience
• Experience of working as a clinical pharmacist at a high level in PN
• Proven experience of working at a strategic level with senior clinicians and managers and experience of working in an integrated directorate team
• Experience and proven ability in project manangement
Desirable criteria
• Good information technology skill
• Experience of implementing CIP or other cost reduction programmes
