Full Job Description

As a Planned Care Clinical Lead, your responsibilities will vary. You will

  • Lead a team of healthcare practitioners some in specialist roles to deliver an effective safe medical service within the prison.
  • Ensuring patients have medical plans of care.
  • Ability to function autonomously and within your sphere of competency, demonstrating effective team leadership.
  • Managing a team of healthcare practitioners.
  • Responsible for appraising the team.
  • You must be able to assess patients, carry out relevant clinical investigations, and arrange follow-up appointments whilst offering advice and giving information.
  • Have an adaptive communication style that enables effective communication with the senior clinical team, the prison staff and external stakeholders.
  • Must be able to clinically evaluate and ensure the safe transfer of patients to acute hospitals, as well as ensuring that plans of care are understood by the patient and the prison as appropriate.
  • You must be able to demonstrate critical thinking skills in clinical decision making.

    Registered nurse Adult
  • Current professional qualification (NMC or HCPC)
  • Evidence of Continued Professional Development
  • Understanding of the complexities of multi-disciplinary and multi-agency working
  • A non-judgmental and compassionate approach
  • Understanding of clinical governance, health and safety, QOF, EBP and national standards
  • Understanding of audit and datix
  • Although not essential ideal applicants would have experience in a hospital, community or prison environment.

    This position is full time, youll receive an annual salary up to £63119 per annum - depending on experience. How will we support you?
