Full Job Description

We are looking for an enthusiastic and motivated Band 6 Physiotherapist to join our dynamic Responsive Services Team. This is an exciting opportunity to work within a fast-paced, multidisciplinary environment, delivering urgent assessment, intervention, and rehabilitation to patients in the community. Our Responsive Services aim to prevent unnecessary hospital admissions, facilitate early discharge, and support patients to remain safely at home. You will work closely with a wide range of professionals, including occupational therapists, nurses, paramedics, rehabilitation assistants and social care colleagues. Responsive Services is made up from Intermediate Care Team, Urgent Community Response, Virtual ward and the Referral Hub.,

  • Provide timely, high-quality physiotherapy assessment and intervention to patients in their own homes or community settings
  • Manage a complex and varied caseload
  • Support hospital avoidance and early supported discharge pathways
  • Deliver rehabilitation programmes to improve mobility, independence, and quality of life
  • Work collaboratively within a multidisciplinary team to develop person-centred care plans
  • Provide clinical advice and guidance to junior staff, support workers, and students
  • Participate in service development and quality improvement initiatives

    HCPC registered with a degree in Physiotherapy
  • Competent in managing acute presentations and complex patients
  • Confident in autonomous decision-making and lone working
  • Skilled in assessing mobility, falls risk, and functional ability
  • A strong communicator with excellent interpersonal skills
  • Flexible, proactive, and responsive to service demands
  • The service runs from 8am-8pm 7 days a week, 365 days a year, the post holder will be expected to work on a rota basis. Due to the nature of the role it is essential applicants are able to drive, have a clean driving license and a car suitable for business purposes.
