Are you looking to join a dedicated and caring team who support local patients when they need it the most? We have a great opportunity for a part time Physiotherapist to join our Patient Flow team to provide compassionate, person-centred, holistic rehabilitation services to palliative care patients. This is a key role that supports our patients, helping to improve their mobility, independence and quality of life both in an inpatient and community setting. You don't need to have palliative care experience we welcome applications from a range of clinical areas. This is a part-time position working 22.5 hours per week and we are open to discussing the working days at interview however it is expected that it will be between Monday to Friday.Main duties of the job This role will cover:

  • Undertaking holistic, special assessments of patients and develop dynamic treatment plans which focus on maximising function.
  • Providing education and support to carers when planning for a patient to be transferred from the IPU to another location and be actively involved in developing treatments plans around self-management.
  • Regularly attending MDT meetings, family meetings and case meetings to provide specialist advice.
  • Keeping up to date patients notes and records in accordance with Hospice policy., We do not have sponsorship status and therefore all our offers are made conditional upon our candidates submitting evidence of their right to work in the UK in conjunction with our other pre-employment checks.

    A recognised qualification in physiotherapy and be registered with the HCPC
  • Experience of delivering group based and individual physiotherapy
  • The ability to assess the needs of individuals and groups including the whole family in order to work with them as a unit
  • Excellent communication skills, both verbal and written
  • A flexible, supportive and calm approach to work
