Full Job Description

To work as a Physical Health Care Lead/Non-Medical Prescriber in the Clayfields Health Care Team in the Local Authority Secure Children's Home. Working with a defined autonomous caseload under supervision of the Clayfields Health Team Operational Leader, visiting General Practitioner and Advance Nurse Practitioner. Providing high-quality nursing care, through assessing, planning, implementing and evaluating physical health care plans for young people. Focusing on the public health needs of the population of young people within Clayfields Secure Unit( ages 10-17) and developing the nursing aspects of the healthy school's programme including vaccinations.

To provide appropriate nursing care for young people with regards to their physical health needs, primarily via face-to-face contact. Providing advice and information to Children and Young People in a secure setting, the Health Care team, Local Authority Care staff and other stakeholders as appropriate and required.

· As part of the Clayfields House Health Team providing 1:1 and group interventions and contributing to the delivery of education programmes for children and young people and teaching and training for Clayfields Staff.·

To provide appropriate nursing care for young people with regards to their physical health needs, primarily via face-to-face contact. Providing advice and information to Children and Young People in a secure setting, the Health Care team, Local Authority Care staff and other stakeholder as appropriate and required.

To work with young people who have a wide range of physical health care needs, alongside other complexities of mental health, trauma and attachment difficulties, neurodiversity, communication/sensory needs, substance misuse, health inequalities and social care need., · Deliver a high-quality nursing service to meet the health needs of a defined caseload and contribute to the care of young people with multiple complex needs.

As a Non-Medical Prescriber, medication governance in line with Trust policies and procedures. Work Collaboratively with Clayfields Health care team and Clayfields Care staff.

Collaborate with the GP, Advanced Nurse Practitioner, pharmacy, dental bus and Trust school's vaccination service.

Undertake medication reviews and audits.
