Full Job Description

We are looking for an Physical Health Support Worker Lead to join our team at Cygnet Hospital Ealing who is experienced of working within General hospital outpatient clinics. You will be expected to work 40 hrs a week (Monday Friday) (9am 5pm) As a Physical Health Lead Support Worker, your responsibilities will vary. You will be involved in embedding physical health care policy and promotion, along with supporting a GP clinic, Clozapine clinic and other physical health interventions within the hospital. You will support and assist patients with their individual physical health care plans, as well as being a pivotal team member in the input into these. This is an exciting, hands-on role, you may be taking bloods or providing support to a service user who is anxious about their physical health. Examples of some day to day tasks may be, monitoring patients and performing basic health checks, coordinating a GP clinic and leading a health awareness session with staff.
