Physical Health Nurse

• Lead on patients physical healthcare assessments, care planning and ongoing reviews.
• Provide specialist advice on areas such as diabetes, hypertension, COPD and wound care.
• Lead Infection Prevention & Control across the site, including audits and outbreak management.
• Deliver physical healthcare interventions including ECGs, phlebotomy and wound care.
• Liaise with primary care services to coordinate patients ongoing healthcare needs.
• Support and train nursing staff in physical healthcare, observations and clinical assessments.
• Lead health promotion and smoking cessation initiatives.
