Full Job Description
We are looking for a motivated and dedicated Pharmacy Technician to join our primary care team at [Practice/PCN Name]. This is an excellent opportunity for a registered pharmacy technician to develop their skills within a supportive, patient-focused environment, contributing to the safe and effective use of medicines for our practice population. You will work closely with both practice-based and remote clinical pharmacists, GPs, nurses, and the wider primary care team to support medicines optimisation, repeat prescribing processes, and high-quality pharmaceutical care for patients in the community.,
- Manage and process repeat prescription requests accurately and in line with practice protocols
- Support medicines reconciliation following hospital discharge and care transfers
- Conduct structured medication reviews under the supervision of the clinical pharmacist, where competent to do so
- Liaise with community pharmacies, hospitals, and care homes to resolve medicines-related queries
- Support safe and efficient management of practice prescribing systems, including monitoring of high-risk medicines and shared care arrangements
- Provide advice to patients, carers, and healthcare professionals on the safe use of medicines
- Support audits relating to prescribing, medicines safety, and cost-effective prescribing
- Assist with the implementation of national and local prescribing guidance and safety alerts
- Maintain accurate records in line with legal and professional requirements
- Participate in clinical governance, audit, and quality improvement initiatives
- Work flexibly across the practice/PCN as needed
