Full Job Description
The Pharmacy Technician will work as part of the multidisciplinary team within the General Practice to support safe, effective, and efficient use of medicines. They will assist with medicines optimisation, repeat prescribing processes, patient education, and clinical administrative tasks under the supervision of a clinical pharmacist and GPs., Medicines Optimisation
Conduct medication reviews.
Review patient compliance and support adherence through education and advice.
Monitor high-risk medicines and ensure required blood tests and safety checks are up to date.
Undertake regular audits that pertain to patient safety.
Repeat Prescribing & Medicines Management
Process repeat prescription requests in line with practice protocols.
Update patient medication records, reconcile medicines, and identify discrepancies.
Manage acute medication requests and queries, escalating appropriately.
Support safe transfer of care by reconciling discharge summaries, clinic letters, and hospital medication changes.
Patient Support & Communication
Provide information and promote understanding of medicines to patients, carers, and care homes.
Advise on safe storage, disposal, and correct use of medicines.
Respond to patient queries related to prescriptions and medication availability.
Clinical Systems & Administrative Support
Use GP clinical systems SystmOne to update medication records and read codes.
Contribute to the development and implementation of practice protocols and policies around medicines.
Collaborative Working
Work closely with GPs, clinical pharmacists, nurses, reception team, PCN staff, and community pharmacies., Nursing Team:3Registered Nurses and 1 Nurse Associate ensuring continuity of care
Healthcare Assistants:3 HCAs supporting patient care and clinical efficiency
Pharmacy Team:6team members that focus on optimizing medication therapy, working directly with patients and healthcare teams to improve outcomes and safety.
Admin and Care Navigator Team:14 team members that provide essential support within the GP practice, combining administrative efficiency with patient focused navigation to ensure smooth access to care.
Together, this multidisciplinary team works closely to provide comprehensive healthcare services, with a strong emphasis on teamwork, innovation, and professional development.
