If youre passionate about medicines optimisation, patient safety, and shaping innovative primary care services, wed love to hear from you., As a Practice Pharmacist, you will deliver patient-centred clinical care through structured medication reviews, support the safe and effective management of long-term conditions, and provide expert advice on medicines to both patients and the wider practice team. You will manage repeat prescriptions, carry out medicines reconciliation, and identify and resolve prescribing issues, while contributing to patient safety, clinical audits, and quality improvement initiatives within the practice.
