As a Clinical Pharmacist, you'll be an integral part of the multidisciplinary team, working closely with consultants, nurses, and healthcare professionals to ensure patients receive safe, effective, and personalised medicines management throughout their hospital journey. Unlike traditional pharmacy roles, you'll spend most of your time on the wards, interacting directly with patients and influencing clinical decision-making, rather than working primarily in dispensary services., Deliver a high-quality clinical pharmacy service across the hospital Review medication histories, prescriptions, and clinical information to optimise patient outcomes. Provide expert medicines advice and support to consultants, nurses, and wider clinical teams. Support patients with discharge medicines, side effect counselling, and medication adherence. Promote the safe, appropriate, and cost-effective use of medicines. Identify opportunities to improve processes, enhance patient safety, and drive service development. Build strong collaborative relationships with colleagues across multiple specialties. Contribute to a patient-centred culture focused on exceptional care and outcomes.
