Full Job Description

We are looking to recruit to an already existing team for the out of hours Hospital to Home Team on a zero hours basis. This is an excellent opportunity to work within a dynamic team contributing to and supporting all aspects of getting people home from hospital, in a safe manner, whilst settling them in their home to promote a healthy recovery after being in hospital. This supporting the NHS, Public Health and our Fire Service strategies in making communities safer within Warwickshire. This role involves completing a Safe and Well Check when returning people home and making the appropriate referrals where required. About the Role We are looking for highly motivated individuals who will assist in the delivery of Hospital to Home and Prevention activities; working collaboratively to drive change internally and further strengthen our Health partnerships as part of the PPR Strategy. As part of the role you must be committed to Safeguarding Vulnerable Adults whilst carrying out your work. You will also be required to undergo an enhanced DBS check. We would also expect you to uphold the principles of the Core Code of Ethics. There are 2 teams currently in place for the out of hours team, 1 based from Nuneaton Fire Station and another from Leamington Fire Station. The teams consist of 2 team members, who are required to respond to one of the base locations within 40 minutes of being mobilised by Control. Staff are required to be able to respond within shifts that are 4 hour blocks. This includes the following;

  • Weekdays -18:00hrs until 22:00hrs (Control will stop taking calls from Hospitals at 19:00hrs)
  • Bank Holidays-10:00hrs-14:00hrs, 14:00hrs-18:00hrs and 18:00hrs-22:00hrs (Control will stop taking calls from Hospitals at 18:00hrs), The Fluency Duty is outlined in the Code of Practice on the English language requirements for public sector workers. The Fluency Duty for this role is Required. You must be able to speak and provide advice in accurate spoken English to the public.
