Full Job Description
You will join the established team as an experienced Community Psychiatric Nurse, Occupational Therapist or Social Worker and will work across our Care Coordination and Initial Assessment team delivering timely, accessible services to women and families through a community-based model of perinatal mental health care. You will ensure, in collaboration with other clinical colleagues, that professional practice meets standards as set by statute, regulatory and other professional bodies and HPFT.,
- To work as an autonomous practitioner within the Community Perinatal Team, carrying a caseload of women with moderate to severe mental health needs during pregnancy and the postnatal period, providing specialist perinatal mental health assessment, care planning and ongoing intervention.
- To work collaboratively with maternity services, health visiting, primary care, adult mental health services, children's services and other partner agencies, contributing to joint working, information sharing and safeguarding processes where required.
- To practice in accordance with professional codes of conduct, Trust policies and perinatal service protocols, maintaining up-to-date clinical knowledge and skills through supervision, training and reflective practice., To carry out telephone triage screening assessments, routine initial assessments and respond to urgent referrals carrying out comprehensive assessments where clinically indicated.
- To provide brief follow-up interventions whilst awaiting allocation to another member of the team's caseload
- To contribute to the team's duty system on a rota basis.
- To plan, organise and prioritise resources and tasks on a daily basis to meet service and organisational needs.
- To ensure a consistent and timely response to safeguarding cases in line with Trust procedures.
- To take a leading role in the day to day running of the service, supporting the Team Leader.
- To work in conjunction with and alongside colleagues and partners within the organisation.
- To plan and deliver treatment interventions in line with best practice and local and national Guidelines.To take a leading role in designated projects/service development as allocated.
- To support the Team Leader in the effective and timely management of complaints and serious untoward incidents.
- To plan, develop and deliver training to others as needed.
- To work collaboratively with colleagues in Primary, Secondary and Tertiary Care to improve existing service provision within the Trust thus enabling the effective management of care within the community, avoiding in-patient admissions whenever possible
Registered Nurse (Mental Health/Learning Disability) Social Worker or Occupational Therapist - Teaching certificate including annual update (ENB 998), or equivalent.
- Evidence of CPD Desirable criteria
- MSc in specific therapeutic treatment modality e.g. systemic practice, cognitive behavioural therapy.
- Post graduate clinical and therapeutic qualification related to the community., Sound experience of working in a range of settings relevant to the post
- Experience of working with service users presenting with emotional/psychological difficulties and challenging behaviour with their families and wider networks in situations of crisis and emotionally demanding environments.
- Experience and knowledge of implementing safeguarding procedures for children and adults
- Experience and knowledge of working in a multi-ethnic community Desirable criteria
- Experience of therapeutic practice in perinatal mental health.
- Experience of teaching and training in practice.
- Experience of peer supervision, supervising junior colleagues and consultation in other agency settings.
